Request CM comforter and journal for a woman with cancer

If you know of a woman diagnosed with cancer whom you would like to receive a CM comforter and journal you may contact Comforter Ministry, Inc. and request they be sent to her, providing the recipient's name and complete mailing address. You may download CM Participation Form and send via mail or complete the online form below.
A cover letter will be included, stating you are the one requesting they be sent.
A brief explanation of the Comforter Ministry and what to do with the comforter and journal are also included in the cover letter.
Comforter, journal and cover letter are boxed and sent via USPS. Insurance and delivery confirmation costs are added.
The recipient will be listed on the Recipient Prayer List by first name, City and State for the Comforter Ministry prayer warriors to include them in their prayers.
